YEARLY MEMBERSHIP PLAN
PURPOSE
To give patients without insurance the opportunity to receive dental care at affordable prices.
The discount plan provides free exams, prophylaxis cleanings, bitewing X-rays, and fluoride treatments twice a year. Please note that this discount does not apply to products sold or the use of nitrous oxide.
All other treatments will be provided at a 20% discount off regular dental fees, except orthodontic services, which are not eligible for the discount.

TERMS & LIMITATIONS
There are no yearly maximums and waiting periods.
This is an annual contract that does not automatically renew. The patient must notify us if they wish to renew the contract for another year to continue receiving services.
Dental insurance supersedes this contract, In the event you obtain dental insurance, this contract will be null and void. The contract can be terminated at 6 month intervals if you move out of the area or are not satisfied.
Co-pays are due at the time of service.
Discount prices are available only for work performed in our office. Work performed by our referral partners and/or performed in hospitals receive no membership discount.
